    Are you a California resident? Yes No

    Will you be a full time student (12 units) Yes No

    Did you test into foundational classes (Reading, Arithmetic, Writing)
    on the Assessment Test? Yes No

    Did you qualify for a Board of Governors Waiver A or B? Yes No

    If you answered "Yes" to all of the above questions, you are eligible to apply for the Extended Opportunity Program and Services. The number of participants is limited, so apply early.

    Priorities of Acceptance:


    1. EOPS eligible single parent on CalWORKs with one child under the age of 14

    2. EOPS eligible with three foundational areas of study and returning EOPS students in good standing

    3. EOPS eligible with two foundational areas of study

    4. EOPS eligible with less than 30 degree applicable units

    This is the priority in which students will be accepted into the program.
