Retirement

California State Teachers' Retirement System is a defined benefit program and is mandatory for all full-time faculty and educational administrators. The District and the employee contribute to CalSTRS.


California Public Employees Retirement System is a defined benefit program. All full-time classified and part-time classified employees meeting the eligibility requirements are enrolled in the miscellaneous employees plan. The District and the employee contribute to CalPERS.

National Benefit Services, Inc. (NBS) is a fee for service firm specializing in the design and administration of all types of Retirement Plans, Cafeteria Plans, HRA's, HSA's, & COBRA Administration. Health Plan Description

Medical
Blue Cross

Option 1

HMO (Health Maintenance Organization)
The HMO plan has no deductibles. Subscribers are required to select a primary care physician who provides or coordinates health care services. There is a $20.00 co-payment per visit but no deductibles.

Option 2

PPO (Preferred Provider Option)
For PPO service there is a guaranteed payment to doctors (90% of Negotiated Rates paid by Blue Cross, 10% paid by employee) with a $350 deductible per person/$700 per family per calendar year. Out-of-network providers are guaranteed payment (60% of Eligible Charges by Blue Cross, 40% by Employee) with a $700 deductible per person/$1,400 per family per calendar year.



Prescription

Medco Health Solutions, Inc. - Brand Name co-pay $15 and Generic co-pay $5 at participating pharmacies for a 30-day supply. The Mail Service Prescription Program for ongoing maintenance drug prescriptions is $30 co-pay for Brand Name and $10 co-pay for Generic drugs for a 90-day supply.



Dental

Delta Dental Plan of California - Deductible of $50 per person/$150 per family per calendar year. The maximum paid per calendar year is $2,000 per person for DPO Providers and $1,500 per person for Non-DPO Providers. In-network dentists are covered for 100% of approved basic, diagnostic and preventive benefits. Out-of-network dentists are covered for 80% of Delta approved fees.



Vision

Vision Service Plan (VSP) - The Plan allows a vision examination every 12 months, lenses every 12 months, if needed, and frames every 24 months, if needed. The VSP is a preferred provider plan and their services are guaranteed, if provider eye doctors are utilized. There is a $15 deductible per calendar year. Lesser amounts are paid if non-VSP providers are used.



Mental Health

PacifiCare Behavioral Health - Inpatient Annual Maximum Benefit: Unlimited days at 100% coverage. In-Network: No Charge. Out-of-Network: Not covered. Outpatient: In-Network: 50 visits at $0 co-payment. Out-of-Network: Maximum of 40 visits per calendar year 50% Usual Customary Rate up to $40 per visit.



Life Insurance

Standard of Oregon Life Insurance - A $20,000 Life Insurance/$20,000 Accidental Death & Dismemberment plan is included in the District's benefit package. The policy is in effect as long as the employee is an active, benefit eligible employee. The benefit amount is automatically reduced to $13,000 once a current employee reaches age 70 and $10,000 at age 75. Classified employees who work less than 20 hours per week, and persons employed in short-term assignments are not eligible for the group life insurance benefit.
